BANK FEED APPLICATION FORM

Hello

I am trying to set up bank feeds for a company credit card.  Due to the fact that for some reason, the company credit card had to be set up under the employees personal profile (WBC requirement), the company has third party access to the card account as they are paying for the card expenses.

I went to try and set up bank feeds through MYOB and when I am directed to the bank website to complete this, the card doesn't show in the list of accounts and therefore I am unable to do apply for the bank feeds.  I have done something similar in the past and somehow received a bank feed application form from MYOB.  I have looked everywhere for this but can't find it.  Do I need to fill in the form rather than go through the bank website, and if so, where do I find the form?
