Auto fill Client information into document or form

Is there a way I can auto fill a document or form with a pre exisitig clients information.  I want to create a document/ form and then I can select a client and it will automatically insert the info...

Any ideas? Anyone?
I have tried the live chat feature but it just keeps ending the chat by itself.
