This is a known offline file issue in MYOB AccountRight, not user error.
When you click Record, the transaction fails to write, so the invoice number does not move and the entry disappears. Item creation still works because it saves to a different table.
If cache clearing and reinstall did not help, the file likely needs MYOB backend repair. Restoring backups only works briefly, which matches what others are seeing.
Best next step is to log a case with MYOB and ask for offline file repair or escalation for AccountRight 25.11. This is a blocking bug, not a workaround issue.
