Macquarie Bank (Aus) bank feed authority form

Hi, I'm trying to set up some feeds with this bank and it requires that I download a form to sign. When I click the button to download, nothing happens (i.e. there's no form downloaded), or I'm getting an error page suggesting I reload the page. Then I have to start the feed request all over again. Any ideas?
