Shipping Address Phone Numbers

Question

We have used the ship to addresses for multiple locations and have added phone numbers for each of these locations to appear on the invoices and packing slips however, it only picks up the phone number from the first ship to address. How can change this so that it picks up the phone number that matches the ship to address when it prints out?

In order to display the phone numbers for other addresses, please choose the relevant phone number fields in the custom form as shown below :

Does this mean that the phone number fields need to be added to the form as separate fields?

If so, this crowds the form to have all these fields added on one form. Is there a simpler way where the fields can be combined on the form and it picks up the one listed?
