I didn't think it was possible to entirely delete items that have appeared on an invoice or other doctype, is that still the case?

It can be done, however only with specialist software, which FGH Tables has access to.

I had a feeling that was the case.
I suppose the best way to check the company file size is to download a backup copy, correct? And then we should assume it's taking up about 30% more space online, correct?

Thanks gavin12345, for sharing insights on this issue. Regarding your concern on the company file size, yes, you make a backup of the online file and restore this to your local Library.
