Thanks for your post. If you go to Print/Email Remittance Advices and click on Advanced Filters... you should be able to select the customised form you created. Make sure that you don't have any forms with duplicate names.
If you are still having issues, could you clear the AccountRight cache, try again and tell me exactly the steps you are taking, ideally with accompanying screenshots?
