Transfer of subscription

Question

I am changing accountants and I currently have my subscription with them for MYOB. I have read on the forum that I need to complete a form to initiate the transfer process from them. I would appreciate if you could email me this form

Here's the Change of Details form to transfer your MYOB subscription from your current accountant. Once you've filled it out, just email it to customer_service@myob.com for processing.
